Plague doctors are medics that go around attempting to heal those that have been inflicted with the plague. They usually wear a leather gown, gloves, a leather mask with red glass eyes, a beak filled with fragrant herbs and oils, and a leather wide-brimmed hat to indicate that they are doctors. Underneath the gown, they wear leather breeches and a leather shirt. The costume is to protect themselves from the sickly and those who have gone insane from disease. They also carry an arsenal of tools for amputation, cyst popping, and healing. The plague doctor, while not a true healer, is by far one of the most competent healers for their era.
The plague doctor is an archetype of the medic class.
Plague Doctor’s Outfit
At 1st level, a plague doctor gains a unique mask. While wearing this mask, he gains dark vision up to 60 feet or increases his own darkvision by 30 feet. In addition, he can detect diseases and poisons up to 60 feet. This mask takes up his head and eyes slots. This mask costs the plague doctor 20 gil to replace if damaged beyond repair.

Summon Swarm (Su)
At 3rd level, a plague doctor can summon a swarm of bats, rats, or spiders (his choice), which attacks all other creatures within its area for a duration of 1 round per medic level. (He may summon the swarm so that it shares the area of other creatures.) If no living creatures are within its area, the swarm attacks or pursues the nearest creature as best it can. The plague doctor has no control over its target or direction of travel. He can use this ability a number of times per day equal to his Wisdom modifier (minimum 1).

Transfer the Unwell (Su)
At 4th level, a plague doctor is able transfer any poison, disease, sicken condition, or the nauseated condition from another creature to himself and is able to transfer it to another creature. The plague doctor is not affected by any effects. If the effect has a duration, it pauses temporarily until the plague doctor transfer it to another creature. To transfer, the plague doctor must make a melee touch attack and the creature is now affected by it (no save). The creature can make normal saves (if it is allowed to) for the rest of the duration. The plague doctor can only transfer 1 status effect or condition at a time.
This ability replaces healing hands.
Miasma (Ex)
Starting at 5th level, if a plague doctor is adjacent to a creature at the beginning of its turn, the creature must succeed at a Fortitude save with a DC of 10 + half the medic’s level + his Wisdom modifier or become sickened for 1 round. A creature of the animal, fey, or plant type that fails its save is nauseated for 1 round and sickened for 1 minute thereafter. If the creature makes it’s save, it is immune to this effect for 24 hours, as are creatures immune to disease.
This ability replaces injections.
Aura of Putrefaction (Su)
At 6th level, a plague doctor exudes an aura of disease and filth that degrades his enemies. All enemies within 10 feet to take a –4 penalty on saving throws against disease and poison. Creatures that are normally immune to disease or poison lose that immunity but don’t suffer the penalty while within 10 feet of the plague doctor with this ability.
At 8th level, the plague doctor is immune to diseases, including natural and supernatural diseases, though he can still contract them and spread them.
At 17th level, all allies in the aura gain a +4 circumstance bonus against disease and poison as the plague doctor’s powers direct those effects to others.

Each creature that fails its save (including the original target) takes damage equal to the plague doctor’s Wisdom modifier.
This ability replaces improved injections.
Plaguebearer (Su)
Starting at 14th level, any creature that strikes a plague doctor with a touch attack, unarmed strike, or natural weapon must succeed at a Fortitude save with a DC of 10 + half the medic’s level + his Wisdom modifier or contract a disease, as the contagion spell. If the creature makes it’s save, it is immune to this effect for 24 hours.
This ability replaces improved resuscitation.
Pestilential Breath (Su)
At 15th level, the sickness within the plague doctor’s body finally becomes so potent that the plague doctor’s very breath is deadly. Once per day as a standard action, the plague doctor can exhale a cloud of pestilence in a 30-foot cone. Those caught in the area of this miasmic cloud receive a single Fortitude save to avoid suffering the effects of two different diseases. The DC of this save is equal to 10 + half the medic’s level + his Wisdom modifier. The plague doctor can choose what two diseases he inflicts on each target that succumbs to the plague doctor’s plague breath, but they must be two different diseases chosen from the following list: blinding sickness, bubonic plague, cackle fever, filth fever, leprosy, mindfire, red ache, shakes, or slimy doom. The victim suffers the initial effects of these two diseases immediately — use the diseases’ frequency and save DC normally to determine further effects. At 17th level, the plague doctor can use this ability twice per day. At 20th level the plague doctor can use this ability three times per day.

Plague Carrier (Su)
At 19th level, the plague doctor’s touch inflicts mummy rot on those he strikes. The plague doctor can choose to suppress this ability for 1 round as a swift action. The creature touched can resist contracting mummy rot by making a Fortitude save – the DC is equal to 20 + his Wisdom modifier.
This ability replaces greater resuscitation.
